Experience and knowledge of working with older adults with Mental Health conditions within the including Alzheimer's and Dementia. Prior experience of working in the community would be essential.  Ability to carry out assessment and review of patient’s needs, monitoring of mental well -being and putting effective care plans in place to reduce relapse and hospital admissions.
